    Any idea how I can edit the format of the form? I would like to have several fields per row.

    You can assign css class to any field(by user meta) and then write your own css to style them.

    Can you provide a little detail in your answer? I added class names to all my fields, but am unclear if User Meta creates these styles somewhere and I fill them in, or if I have to add the styles manually to style.css…thanks
