Random fatal error on profile creation with an avatar

We are seeing an issue that fires randomly and we’re not sure why. At least once a day a user will tell us that they got a fatal error on screen when registering for our site and upload their avatar at the same time. We test it, it works great. We test with their artwork, and it works great. We can not seem to figure out why, at random, this will happen.

Below are 2 examples from today:
