2 Issues:
1. First and Last Name (WordPress Fields) are not saving upon new user registration.
2. I was looking at http://user-meta.com/forums/topic/field-on-backend/ and when I created a profile page, with the text [user-meta type='profile' form='profile'], I get error form does not exist. Do I manually have to create a form for profiles or does the plugin do that automatically?
1. First and Last Name should saved upon registration if you use WordPress default field. Please post your url here to let me check.
2. [user-meta type='profile' form='your_form_name'], replace your_form_name with the name of your form you have created from User Meta >> Forms Editor.
I think any other plugin which is using registration hook is conflicting with user mata pro, please try to temporarily disable similar types of plugin and try.

So on the 2nd question I have to create my own profile and login pages? Isn’t the profile page the same as registration?
I guess I am seeking you best advice and recommendations on designing this right. Thanks again for all your help!
You can use same form(being used in registration) or different form for profile page.
