The import is simply not working. Regardless if I create the .csv file from Excel or Numbers or Google Docs, User Meta treats every cell as a CSV Header and so lists each and every cell to be matched to a field. Every cell in the .xls file is set to “Text”.
Ok, got it working. Not exactly sure what I did to be honest but it required stripping some odd characters, importing to mySQL and then generating the .csv file phpMyAdmin… Neither Excel, Numbers nor Google could generate a .csv file that this plugin could parse…VERY ANNOYING.

pretty crazy to have to create a CSV document with mySQL rather than the widely used excel/ google docs
