Can't get rid of the word 'Array' next to checkbox

For some reason the word ‘Array’ appears next to the checkbox on my form. It gives me the value I need when checked, so it seems to be working fine, apart from the above problem. Can someone tell me how to get rid of it as it is unnecessary and looks unprofessional. You can see it here:

Hi,
You can insert yes=Subscribe inside field options. and can remove field title. Or you can simply put yes into field option.
