I had the same issue – it is with version 1.1.5. I could not remove licence nor activate after moving my site to a new host, same url.

Solution was to go back to 1.1.4.1 and reactivate licence – magically worked for me!

Note, I didn’t get any support either…

Thank you for your reply.

1. I copied the form name from the forms editor so I know it is correct.

2. Ok, understand but it doesn’t then give me any clue as to why I can’t select from these particular fields. I did change their field type from radio buttons to multi-select at one stage, but I went through and reentered all the data to ensure the values should be correct. Show that make any difference?

3. What if I delete plugin and its data? Do the wp_usermeta values remain? I am trying to determine what is conflicting here. amr-user developer Anmari imported all my amr-user and usermeta data and it worked fine for her, so there is something odd on my system.

I am still in dev so I can give you a login if you are happy to take a look – might help sort out #1 above?
