Undefined index: user

Hi,

I’ve been having very strange behavior with regards to my users’ activation.

My users who have not logged in after I have manually activated them seem to eventually fall into a state where they are not activated… but not deactivated. I have both “Activate” and “Deactivate” options listed under their username in the Users editing panel.

I have the same behavior whether my User Activation settings are: 1) User auto activation, 2) need e-mail verification, and 3) need admin approval.

Along with this behavior, I’ve been receiving the following PHP notices:
