Form not saving checkbox

Hello.

i have created a form and put it in the frontend via shortcode. It’s only one checkbox, but it doesn’t save the user checking it. When i reload the page after clicking the save button, the checkbox is unchecked again. What is wrong here?

Is the issue still there? Is it possible to provide an example link? It is possible the checkbox is not showing the right value for caching. Please check if any plugin or script caching the initial value. We are also testing the possible scenario where this can happen.
