About 5% of the time, I and users are unable to log in from User Meta Pro. Login always works from wp-login.php, which I leave open since UMP is not 100% reliable. This has been happening for a long time (1+ years), but I really tried to focus on what might be the cause today. Oddly, if you do login via wp-login.php, then log out, and then login via UMP, it seems to always work. Its’ somewhat hard to get to fail on command, but once it fails, no logins work (I have a group I can use for testing).
In the failed state, you enter the username and login, often from Chrome’s password cache, you get the spinning “thinking” graphic for about 5 seconds, and then the grpahic it disappears. There are no displayed messages on-screen. Not even a generic “your password is not valid” kind of message.
Ok, I caught two php errors in UMP that may provide a clue:

On the warning, it’s a bit odd in that my site is not a multisite, but perhaps the same code is used for all sites. I’m fine with php if you need me to swap out a file or add error_log lines to help work it out.
