Install User Meta Pro

if you have free version installed, go to your WordPress Plugins page and deactivate User Meta. Then select Delete or go to your web server (via FTP, SSH, SFTP, etc.) and delete the entire user-meta folder. (Deleting the plugin won’t effect on your saved fields/forms or settings)
Go to Plugins >> Add New from admin panel. Click upload from upper menu. Locate user-meta-pro.x.y.z.zip file and click Install Now. Plugin will start uploading, after complete uploading, Click Active Plugin. (For ftp: unzip user-meta-pro.x.y.z.zip, upload user-meta folder to wp-content and activate the plugin from WordPress Plugins page).
